I spent a great deal of time trying to figure out why I was seeing the same issue as above, and then realized the enemy NPC was standing in an unlit area and being considered hidden. I wish there was a way for the DM to see more info on why adv, dis etc are being pulled on the info card.

When I use enemies like Swarms that are resistant to all physical damage, it works fine.
